Consent/Authorization

    I hereby consent to the conduct of Northpoint Development Bank, Inc. of gathering/ collecting, processing, use, retention, sharing and disclosure of my personal information or data, whether classified as personal, sensitive personal, or privileged information, as may be necessary or incidental in the evaluation/ processing of my deposit by way of compliance to the Republic Act No. 10173 known as the Data Privacy Act (DPA) of 2012 and other related laws and issuances.

    I waive my right to confidentiality of information regarding any and all matters that I have voluntary disclosed or indicated in any written form, including submitted documents and I further authorize Northpoint Development Bank, Inc. to check the authenticity of the documents that I have submitted and conduct verification concerning the same, as well as to validate whatever data I indicated/ disclosed.
